This plan is per individual and offers 12 visits annually. We offer a 30% discount for Children and adults up to 30 years old. Please download the PDF to get all the health plan details and services.
- The membership covers office visits only
- 50% discount for (in-house medications given by IV and IM, and in-house Laboratory Tests and Procedures).
- 35% discount for sent-out labs.
- 35% discount for (in-house Diagnostic Imaging including X-Ray, Ultrasound and Echocardiogram).
- 10% discount for Sleep apnea test.
3 Months PlanPopular
This plan is designed for individuals and includes 3 visits within 3 months. It is the most popular plan with the ability to cancel after 3 months.
